To continue your benefits once you are approved by the V.A. you must provide the Office of Financial Aid with the following documents:

  • A copy of your registration receipt -- these will reflect your class schedule, along with tuition and fee charge.
  • A letter from your academic advisor approving any course substitutions. You can only receive payment for courses which apply to your degree plan.
  • A copy of any course schedule changes, regardless of the time left in the semester -- failure to provide us with this form may result in your losing eligibility or may require that you repay a portion of the benefits you have already received.
  • If you change your major, leave school for more than one academic year, or if you complete an Undergraduate degree and enroll in a Graduate program, you must complete a new form 22-1995 and be re-approved by the Veteran's Administration.

All forms referenced here may be completed online on VONAPP and printed for submission to our office.  If you use VONAPP, your application will be submitted to the Department of Veterans Affairs but not to our office.  Please print an additional copy to submit to us with any required supporting documents as indicated above.

If you are unable to complete the application online, you may click on the name of the form above to download, print, and complete by hand.  You must submit copies to the Department of Veterans Affairs and our office. Adobe Reader is required to download forms.
