Software Removal Request Form

Please note: One form must be completed for each unique piece of software being requested for removal.

Removal Information*:

Please note: The asset tag number is the six digit number found on the white sticker affixed to the computer. This number is issued by Texas Wesleyan and usually begins with "0".

Dean/Supervisor Approval*:

Have you gained approval from the dean or supervisor of the identified software user to have the software removed?

Please note: If you indicated "No", the form cannot be processed. The requestor must obtain permission from the software user's dean/supervisor prior to submitting a software removal request. For assistance please contact the appropriate supervisor.
